Medical care and emergency hospital care during this Covid pandemic are very different than they used to be. Now, in many settings, if you or a loved one need to go to a medical appointment or to the hospital, you will not be allowed to be accompanied by a family member unless you are delivering a baby or are a child under 18 years old.  

To avoid any confusions and unnecessary delays in receiving prompt medical care, prepare and carry with you in your wallet or pocketbook a medical care document listing the following ...

• Name
• Date of Birth
• Allergies  
• Medications
• Emergency Contact Info/Health Care Proxy Contact Info
